• Complete the ROI, located in the CTS word templates
  • Additional language translations are at the bottom of the list and start with "X"
• Collect the person's written signature
  • You can do this in-person, via encrypted email, secure upload site, or mailed back and forth.

• Get verbal consent over a recorded phone call to share with the other party
  • These should be very time limited (today/tomorrow), specific to whom and what will be communicated.
  • Three-way calls are also an option.
• If you cannot get an ROI or verbal consent, do not share or request information with anyone other than the person you are serving.
• Provide resources to the person to call on their own.

• If there are any changes to the ROI (i.e. adding a new party), update the ROI and get a new signature.
• If you continue to need to exchange information with other parties after one year, complete an updated ROI to cover the next year. The ROI expires annually from the date of signature.
  • If you no longer need to exchange information with that other party, the ROI should not be renewed.
