1. Find the record in MinnesotaHelp.info that needs to be updated or corrected. 
2. Open the record with the error or that needs to be updated.
3. Click the Report Data Problem button.
4. Fill out the form.
   1. If you clicked the Report Data Problem button to get to this portal, this field will automatically be filled out with the exact steps you used to get to the record. Leave this field alone. 
5.  Report the issue in the Description of issue field. Provide as much information as possible. 
6. When you are done, click submit. You will receive an email if you entered your email address. 
   
   *If you need to report a data problem to DMP for a record that does not exist, you can access the portal through iSpring's quick links. 

PAS staff should continue to alert DMP if there are changes to a fax number, email, or facility name for nursing facilities using the DMP-PAS Updates Chat in Genesys as this information requires updates to CTS records.
