 When to use: 

When a caller has a question related to creating a will, getting a Power of Attorney, naming a health care agent, letting their doctor know how they want to be treated at the end-of-life, "getting their paperwork in order," etc., and when helping a caller or caregiver get organized to avoid crisis decision-making.
