Power of Attorney - gives another adult the legal authority to act on behalf of another person (i.e., handle their finances and property).
To protect the clients and staff, you should ask each time if there have been any changes with the POA.

 Proof of POA needed when: 

• Sharing any information about the client to another person
• Enrolling in a Medicare Plan

 Before working with a POA: 

• You will need proof the person has the stated authority
• POAs can be received through the Senior LinkAge Line Upload Site

 Callers who cannot provide POA: 

• Do not share any private information about any client unless you get verbal permission or a Release of Information (ROI) from the client or you can confirm guardianship. 
• You can speak in general, broad terms, such as providing general resources.
• If caller has a specific question, you can answer broadly without providing specifics about any client (for example, "typically in the situation you are describing, here are things most people would want to consider...")

Health Care Directive - lets them leave instructions about their health care and to name the person that they want to make health care decisions for them, if they can’t speak for themself.
